This hospice company was born through conversations with each other about our

wildest hopes and dreams about caring for our patients differently.

And one day, our vision became a reality. A reality started with dreams and built on

hard work, focus and a common goal: to be the very best at what we do.

We provide exceptional care for our community by placing dignity and

compassion at the heart of how we care for our patients

in the last chapter of their lives.

Adam Roach, RN, BSN

Executive Director

Adam Roach, RN, BSN was born and raised in Melbourne Australia. He moved to the United States 21 years ago to experience travel nursing in hospital emergency rooms, and found his home in Albuquerque NM 2 years later. He has over 15 years of Hospice experience and feels he has found his life’s passion. That is to support patients, families and caregivers in navigating and understanding the end of life journey. Adam’s goal is to provide our community with the premier Hospice service. He has worked in multiple roles during his tenure and has over 12 years experience as Executive Director. Adam feels fortunate to work with a strong community network which he has built during his time in hospice care.

Melanie Theriot, RN

Director of Clinical Services

Melanie Theriot, RN moved to Albuquerque at the age of 7 and considers herself a native New Mexican. She has worked with the Geriatric population since 2010 as both a Resident Aide and RN. She has been in hospice for the past 7 years as a Nurse Case Manager, Admissions Nurse, Patient Care Manager, and Director of Clinical Services. She is passionate about hospice because this is a time in people’s lives when they need extra support for themselves and their loved ones. She loves that the approach is holistic involving multiple team members and that the person under care is in charge. Everyone deserves compassion and dignity, and it is her goal to see that each person receives it. Melanie is especially skilled at educating and supporting her team members.

Rachel Rankin, MD

Medical Director

Board Certified in Hospice and Palliative Medicine

Rachel Rankin, MD was born and raised in Albuquerque, NM. She received her medical degree from Loyola Stritch School of Medicine in Chicago, IL and did her residency training in Family Medicine in Denver, CO at St. Jospeh's Hospital. She practiced Family Medicine as an outpatient doctor for almost a decade in rural southern Colorado and in Bernalillo, NM before going to UNM for her Hospice and Palliative Medicine Fellowship in 2018. She has found a passion for hospice and palliative medicine and has worked doing both since she finished fellowship. Dr. Rankin is thrilled to have the opportunity to work with this team to serve the Albuquerque area community to offer the very best hospice care available,

Daniel Aaron James, RN

Director of Quality Improvement

Daniel A James, RN moved to Albuquerque in 1998 and found his new home. While Daniel enjoyed making a difference in the acute care setting such as the Emergency Department, the Medical and Cardiac ICU, and enjoyed Flight Nursing for many years, he found his true calling in 2018 when he joined Adam and Melanie in providing hospice care for the New Mexico Community. Hospice Care brought out the compassion, drive and fulfillment that revitalized his purpose and calling for the New Mexico Community. He found that he was passionate about providing exceptional care and going the extra mile to make sure every patient's limited time was as comfortable and meaningful as possible for them and for their loved ones.
